We aim to respond to each request within the 40 day legal limit from receipt of written request.

Information we hold about you

The Data Protection Act 1998 deals with information held about living individuals. The Act gives the data subject (the person the data is about) certain rights regarding their personal data. Under the act we must comply with eight principles when handling personal information.

Why we record your details

Your personal information is held by us and other organisations to help us work more efficiently together on your behalf.

The information we hold could help us see if we can offer you a service, to make sure you receive the best care, to help us plan and develop future services and to keep a record of what we have talked with you about regarding your concerns, care and needs.

Information we share with others

Some of the information we hold is sensitive and personal. Under the Act we have a duty to protect your records and keep what you tell us safe.

Our staff, and people who work closely with us, value your rights to keep your information confidential and only to let other professionals know when they have a clear need to as part of their job.

In some circumstances we are obliged to show information to others. This would include when a person or an organisation has a legal right to the information, such as health professionals or the police.

How to request your personal information

Please fill out our Data Protection request form or contact us in writing.

We aim to respond to each request within the 40 day legal limit from receipt of written request.

Personal data is information about living, identifiable people such as their name, address, date of birth, ethnicity, gender, activities, opinions, lifestyle, background and choices.

You can authorise someone else to get personal information for you, but they must provide clear evidence that they have been given permission by you.

Costs of requests

Most requests are free and we won't charge you for information relevant to your request.

We will only charge you for information where the law sets a charge, and this will only include charges for reproducing the material such as photocopying.
